"Safety Video"#
"Paste a YouTube URL and it will be embedded at the top of your waiver form for participants to watch before signing."
"Safety Video URL (YouTube)" — paste any normal YouTube link. All the usual shapes work:
youtube.com/watch?v=…, youtu.be/…, youtube.com/embed/…, and Shorts.
When PBHQ recognizes the link you'll see "Video detected — it will be embedded on the waiver page."
Leave it blank if you don't have a video. Nothing about the waiver breaks.
"Waiver Text"#
"Write your full waiver and release of liability text. This appears as a scrollable block that participants must read before signing."
Paste your complete release of liability into the "Waiver Text" box. Each line becomes a paragraph.
Formatting: plain text only. No bold, no headings, no bullets, no HTML. Write in paragraphs separated by blank lines.
Signed waivers are frozen#
When someone signs, PBHQ stores a snapshot of your waiver text as it was at that moment. Editing your waiver later never alters a waiver someone already signed — their PDF keeps the text they actually agreed to.
That means you can update your wording whenever you need to without corrupting your records.
"Waiver Options"#
One checkbox:
"Show photo/video release consent checkbox" — "Adds an optional consent checkbox for photo and video usage." On by default.
This adds an optional line to the waiver: "I consent to photos and videos taken during my participation being used for promotional purposes." Players can sign without ticking it, and PBHQ records their answer either way.
Leave it on if you post player photos to social media. Turn it off if you never photograph customers.
What participants fill out#
The page includes a read-only reference card, "What participants will fill out":
Full legal name · Date of birth · Home address · Phone & email ·
Minor / guardian info (if applicable) · Acknowledgment checkboxes ·
Typed signature & printed name · IP address & timestamp (captured automatically)
This list is fixed. You can't add custom questions or remove fields.
Saving#
Click "Save Waiver Settings". You'll see "Waiver settings saved."
Things this page doesn't do#
Worth knowing so you're not hunting for them:
- No waiver expiry. A signed waiver never expires and is never re-requested. A player who signed last year is still cleared.
- No custom questions. The participant form is fixed.
- No minor age setting. Under 18 is automatic and can't be changed — see What customers sign.
- No PDF settings. Layout and storage are handled for you.
